What is Hospice at Home?

Caring for someone at home is a full time job with very little time off. It can be particularly difficult if the person has a life-limiting illness as this places an ever-increasing physical and emotional strain on families and carers.

 

Without help and support carers become exhausted and feel they can no longer cope. Admission to hospital, or hospice, is unavoidable in some cases, but Hospice at Home has been developed especially for those who wish the care to continue at home wherever possible.

 

We have a team of qualified nurses and experienced health care assistants who are available to give you a much-needed break when YOU want it.

 

The amount and type of support provided will depend on your individual needs.

 

It may be just a few hours to allow you time to have your hair done, go shopping, or visit friends, or it may be overnight to give you the chance for the main carer to have a good night’s sleep. The amount of help provided can be adjusted as your needs change.

 

Is Hospice at Home available to me?

Yes, if you are looking after someone with a life limiting-illness who is unable to care for themselves, and you live within Southern Derbyshire or the Nottingham borders.

 

How do I get help?

You should speak to your doctor, district nurse or Macmillan nurse. Alternatively, you may contact Treetops Hospice and we will contact the relevant professionals.

 

How do I know who will be sent to help?

Once it has been agreed what help you need, the Hospice at Home co-ordinator will telephone you on a weekly basis to inform you of the arrangements and who will be providing care. On the day of the planned visit the nurse will telephone you to confirm arrangements and introduce themselves.

 

What do I do if I need more help or need to change the arrangements?

Just contact Treetops coordinator and we will be pleased to discuss this with you.

 

How much does it cost?

Treetops is a registered charity and there is no charge to patients, or their families, for any of its services

 

Remember, we are here to help you. Please don’t wait until you are exhausted before you contact us. There is nothing wrong in asking for help – we all need it sometimes
