HOW TO JOIN
Lottery Joining Form
It is really simple to join the lottery. All players must be 16 years or over.
- Each entry, each week costs just £1. Members can have up to a maximum of 20 entries per week; each additional entry will increase your chances of winning and your contribution to the hospice.
- Complete the form with your details. Select your preferred way to pay; by cheque/postal order, standing order or credit card.
- You can pay by standing order £4 every 4 weeks, £13 every 13 weeks, £26 every 26 weeks or £52 annually. Where possible, we prefer payments by standing order as this means less administration costs and more money for the hospice. However, we do accept other methods of payment for £13, £26 or £52.
- Return the form (with the appropriate payment if applicable) to the Freepost address: Treetops Hospice Lottery, Freepost DY290, Derby Road, Risley, Derbyshire DE72 3JY. Please make cheques payable to Treetops Funding Limited.
- We shall write to you with your unique game number which will be your lottery number for the duration of your membership.
- All winners are contacted in writing within 7 days of the Friday draw.
