DAY CARE REFERRALS
To refer a patient, call 0115 949 1264 and say you have a Day Care referral.
We are accepting new patients to our Day Care Unit in Risley, following the completion of a £1.4 million annexe.
The new-look Day Care Unit has:
- Extended to provide care 5 days a week
- Capacity for up to 25 patients a day
- Weekly specialist neurological day with capacity for 15 patients
- Increased provision of facilities such as Complementary Therapies
Day Care provides the following palliative care:
- Clinical monitoring by a staff of registered nurses and qualified staff
- An opportunity for people to socialise and gain peer support from others and engage in a variety of activities
- Respite for the patient and carers
- Bathing and general personal care
- Art Therapy and Complementary Therapies
- Weekly clinic in palliative medicine
Access Information
The Day Care Unit is open Monday to Friday, 10am to 3.30pm, and offers a freshly cooked lunch with the ability to cater for any special dietary requirements. Transport can be provided by volunteer drivers if required.
Referral Criteria
To be eligible for Day Care, a patient must have:
- A diagnosis of a life-limiting illness that is progressive
- A prognosis of approximately 12 months (exceptions are made for long-term neurological conditions)
Anyone can refer to Day Care. Most referrals come from health professionals such as GP’s and district nurses. We require clinical confirmation of diagnosis and prognosis for those referring themselves or a loved one.
Everyone referred has a home assessment by a member of the Treetops Hospice clinical team and clinical referrers are kept informed of their patient’s progress.
