Advanced Care Planning session for people living with Dementia and their family / carers .

Working in partnership with Derbyshire Community Health Services, the Dementia Palliative Care Nurses, our new Advanced Care Planning informal drop-in session are for discussing the benefits of Advance Care Planning and how to start the process.

Why are we doing these sessions

Advance Care Planning aims to help you understand the range of different ways you can plan now for your future. It is a process designed to help you plan and record your future wishes, including priorities of care with those closest to you. If you complete the Advance Care Plan or some sections, there’s a record of your wishes when you can no longer make or communicate these.

How will it help?

The aim is for you to start thinking about and making your own Advance Care Plan with the help of those around you. It can initially seem difficult to think about, and many people do not know where to start. Once it has been written down, it doesn’t mean it cannot be changed and reviewed as you change, and your thoughts/wishes do.

Support Group Dates

The team will be at the hospice the first Tuesday of every month, from 1.30 pm to 3.30pm:

  • Tuesday, 8 August 2023
  • Tuesday, 5 September 2023
  • Tuesday, 3 October 2023
  • Tuesday, 7 November 2023
  • Tuesday, 5 December 2023

Light refreshments and hot drinks will be available.

If you need support with transport to and from the support group, please let us know. We can identify organisations that may be able to help you.

The monthly drop in sessions is a joint venture between Treetops Hospice and Derbyshire Dementia Palliative Care Team and the local Admiral Nurses.
