Eligibility requirements and referrals process
- For anyone affected by a life-limiting illness or bereavement (patients, their families, friends and carers)
- Available to all adults living in southern Derbyshire and Nottinghamshire
- Also open to healthcare, education and social care professionals
If the patient is well enough to attend our drop-in Wellbeing Café at the hospice in Risley, please encourage them to come along. No appointment is needed, they can drop in to any session.
- For anyone with a diagnosis of a life-limiting illness who cannot access our onsite services
- Wellbeing at Home is available to all adults living in southern Derbyshire and Nottinghamshire
If the patient is well enough to attend our drop-in Wellbeing Café at the hospice in Risley, please encourage them to come along. No appointment is needed, they can drop in to any session.
Our Befriending Service is available to all adults in southern Derbyshire and Nottinghamshire who are isolated or lonely due to:
- A life-limiting illness or frailty
- Caring for someone with a life-limiting illness
- Bereavement
If the patient is well enough to attend our drop-in Wellbeing Café at the hospice in Risley, please encourage them to come along. No appointment is needed, they can drop in to any session.
- Hospice at Home is available to all adults living in Derbyshire and Nottinghamshire
- For anyone with a diagnosis of a life-limiting illness that is progressive
- For anyone with a prognosis of less than six months
If the patient is well enough to attend our drop-in Wellbeing Café at the hospice in Risley, please encourage them to come along. No appointment is needed, they can drop in to any session.
- Available to adults, children and young people (aged 7 to 18yrs), and families who are dealing with a life-limiting illness or bereavement
- Available to all users of our hospice services and people registered with a GP practice in Derby city or Southern Derbyshire
