Wellbeing at Home
Our Wellbeing at Home service offers care and support across Derbyshire and Nottinghamshire, for those who are in their final weeks and wish to stay at home. We can also offer a limited service to provide respite for carers to help over a period of crisis or carer fatigue.
A service in your own home
We will carry out an initial assessment to establish how we can support you in the most appropriate way. One of our registered nurses or healthcare assistants will then visit you at home for a visit of between 1 and 3 1/2 hours. Our co-ordination team will keep you informed as to the support we can offer you, working alongside your community team and GP.

- Discuss your future care planning
- Support carers in crisis with short term respite
- Provide care in the last weeks of life
The Wellbeing at Home Service is available during the day from Monday to Friday, our Hospice at Home team provides support overnight.
