•Treatment will only be carried out with my consent, and I may request the session stop at any time.
•Treetops Hospice, its staff, and volunteers are not liable for the outcome of a complementary therapy session.
•I confirm that the information I have provided is accurate to the best of my knowledge.
•I will avoid alcohol and heavy meals before treatment. If I am unwell or have a fever, I will inform the service to rearrange my session.
