Treetops Hospice is seeking a Palliative Care Nurse to provide high-quality, person-centred palliative care to patients in their own homes. This is a rewarding community-based role where you will support individuals and their families at the most important time of their lives, helping them remain at home with dignity, comfort and choice.
About the Role
You will deliver skilled, holistic nursing care to patients with life-limiting illness, working autonomously and in partnership with GPs, District Nurses and the wider multidisciplinary team. You will also support families, contribute to service coordination, and help ensure a responsive, compassionate service.
Key Responsibilities Provide compassionate end-of-life nursing care in patients’ homes Assess, plan and deliver individualised care, including symptom management Administer medications and manage clinical interventions (e.g. syringe drivers, catheter care) Offer emotional support and guidance to patients and carers. Provide Cover and Support for Wellbeing Services. Work collaboratively with healthcare professionals to provide seamless care Contribute to care coordination and service delivery within the Hospice at Home team Maintain accurate clinical records and uphold professional standards
About You Registered Nurse (RGN) with current NMC registration Minimum 2 years’ post-registration experienc Experience in palliative or end-of-life care Confident working independently in a community setting Excellent communication and compassionate care skills Flexible, resilient, and committed to high-quality care
