About the Role

An exciting opportunity has arisen for a Registered Nurse to join our dedicated Hospice at Home team, providing specialist care to patients with terminal illnesses and invaluable emotional support to their families.
This night role will include a combination of:

  • 1:1 overnight care in patients’ homes, and
  • Working on our Roaming Service, delivering flexible symptom control, assessment, and support across the Hospice at Home service.

This varied role offers both autonomy and support, allowing you to use your clinical judgement to ensure the very best care for each patient.

This is a rewarding community-based role where you will support individuals and their families at the most important time of their lives, helping them remain at home with dignity, comfort and choice. About the Role You will deliver skilled, holistic nursing care to patients with life-limiting illness, working autonomously and in partnership with GPs, District Nurses and the wider multidisciplinary team. You will also support families, contribute to service coordination, and help ensure a responsive, compassionate service. Key Responsibilities Provide compassionate end-of-life nursing care in patients’ homes Assess, plan and deliver individualised care, including symptom management Administer medications and manage clinical interventions (e.g. syringe drivers, catheter care) Offer emotional support and guidance to patients and carers. Provide Cover and Support for Wellbeing Services. Health Care Assistant: Hospice at Home/Roaming Service

We are looking for motivated and passionate persons with previous experience working as a healthcare assistant to join our expanding Hospice at Home Team. The ideal person would already have experience working in the community setting with patients at the end-of-life. The successful applicant will be part of a multi-disciplinary team, providing palliative and end-of-life care to patients and families in the community. Our approach is predominantly one-to-one night shifts, so you will often be working independently using your own initiative in line with patient need and Registered Nurse support, and on occasions you may be required to work as part of a team on the Roaming Service. The applicants need to have NVQ Level 2 or above in Health and Social Care and must be enthusiastic, flexible, and have a passion for palliative and end-of-life care. We are looking for individuals who have excellent communication and interpersonal skills and who demonstrate a caring, empathetic and compassionate nature. We work predominantly across South Derbyshire, covering areas from Swadlincote and up towards Matlock. We particularly encourage applicants who live on the outskirts of South Derbyshire (Ashbourne and Swadlincote), although we welcome people within our catchment area. This role involves working night shifts (22:00-07.00), including weekends and bank holidays. Our night staff currently self-rosters to ensure a good work-life balance is maintained. A full UK driving licence and access to a vehicle are essential, as you will be travelling across the community within the hospice’s catchment area. You will also be expected to drive the Roaming Service vehicle. Treetops Hospice is committed to safeguarding and promoting the welfare of vulnerable adults, children and young people.
