Support and Information Service
Eligibility:
- The service is available to all adults living in southern Derbyshire and Nottinghamshire
- Open to patients, their families, friends and carers
- Also open to healthcare, education and social care professionals
- It is for anyone affected by a life-limiting illness or bereavement

Wellbeing Space
Eligibility:
- The service is available to all adults living in southern Derbyshire and Nottinghamshire
- A diagnosis of a life-limiting illness.
Referral process:
- Health and social care professionals can refer patients by phoning the Wellbeing Team on 0115 949 1264 or sending an e-referral on SystmOne
- People can attend our drop-in Wellbeing Café or Derby drop-in by Treetops for a chat about their situation and how they can be best supported
- Patients and carers can self-refer by calling the Wellbeing Team on 0115 949 1264
Wellbeing At Home
Eligibility:
- The service is available to all adults living in southern Derbyshire and Nottinghamshire
- A diagnosis of a life-limiting illness.
- For people who cannot access our onsite services
Referral process:
- Health and social care professionals can refer patients by phoning the Wellbeing at Home Team on 0115 949 1264 or send an e-referral through SystmOne
- Patients and carers can self-refer by calling the Wellbeing Team on 0115 949 1264
Befriending Service
Eligibility:
This service is available to all adults in southern Derbyshire and Nottinghamshire who are isolated or lonely due to:
- A life-limiting illness or frailty
- Caring for someone with a life-limiting illness
- Bereavement

Hospice at Home
Eligibility:
- The service is available to all adults living in Derbyshire and Nottinghamshire
- A diagnosis of a life-limiting illness that is progressive
- A prognosis of less than 6 months
Referral process:
- Patients and their carers can contact us directly on 0115 949 6940. We will need the name and contact details of the patient's District Nurse or GP.
- Health and social care professionals can refer patients by phoning the Hospice at Home office on 0115 949 6940
Therapeutic Services
Eligibility:
- Therapeutic services are available to adults, children and young people (aged 7 to 18yrs), and families who are dealing with a life-limiting illness or bereavement.
- The service is available to all users of our services and people registered with a GP practice in Derby city or Southern Derbyshire.
Referral process:
- Family members and health, education and social care professionals can refer children and young people by phoning Therapeutic Services on 0115 949 6944
- Access to adult services is by self-referral only by phoning 0115 949 6944
- Please note our opening hours are: 8.30am-4pm Monday-Thursday, 8.30am-3.30pm Friday
